Type 2 Diabetes Mellitus – age-based approval

All of:

  • Diagnosis of type 2 diabetes mellitus
  • Patient is ≥ 10 years of age

Documentation to bring

  • Documentation of type 2 diabetes mellitus diagnosis
  • Patient age ≥ 10 years

Approval and renewal

  • Initial approval: 12 months

Not covered when

  • Weight loss without type 2 diabetes diagnosis
  • Prediabetes/diabetes prevention
  • Metabolic syndrome without type 2 diabetes
  • Concomitant use with other GLP-1 agonists or GLP-1/GIP agonists
  • Type 1 diabetes

Policy note: Automation exists: a Mounjaro claim will auto-adjudicate if the patient meets the lookback for one oral medication for diabetes (not including Rybelsus or single-entity metformin) and patient is ≥ 10 years of age (updated from ≥ 18 years of age effective 01/07/2026). If either condition is not met, full PA criteria apply. Zepbound (tirzepatide) is not included in this policy as it is not indicated for diabetes.
