About this site

This site answers one question for people prescribed a GLP-1 medication: what does my insurance plan actually require to cover it? We publish each insurer's written prior-authorization criteria in plain language, with the date it was last verified, so patients and clinic staff can get a request approved on the first try.

Who we are

Agent Eva AI LLC is an independent company founded by Ben Booth. We build and maintain a structured database of GLP-1 prior-authorization policies, reviewed by our research team against the source documents, and offer the same data to healthcare organizations through an API.

Independence

We are not affiliated with, endorsed by, or sponsored by any health insurance company, pharmacy benefit manager, pharmaceutical manufacturer, or government agency. Where a page links to a manufacturer's cash-pay program, the link is labeled as such and is offered as an option when coverage is denied or pending, never as a recommendation over an approved prior authorization.

How the data is made

Every record is sourced from a payer document, structured into the same fields, and confirmed by a reviewer before it is published. The methodology page explains the process, what a verification date means, and what we don't claim.

Privacy

The card checker reads your insurance card in memory to identify your plan and discards the image. We don't store card images, member IDs, or names. Details are in the privacy policy.
