Policy 00886, effective August 1, 2026, page 11: the weight-loss exclusion applies to members whose contract excludes medications for obesity, weight loss, weight management, or weight maintenance. The policy states that most member contracts exclude this benefit. Page 18 limits the policy approval criteria to cardiovascular risk reduction and MASH; no weight-loss approval checklist is established for contracts with weight-loss benefits.
