Depends on your plan's benefitPrior Authorization is only available for members with plan benefit coverage of weight loss drugs. Medications for the treatment of weight loss are specifically excluded as a covered benefit for many plans. OSA indication falls under this weight loss GLP-1 policy.
What you need to qualify
- Age 18 and older
- BMI of 30 or higher
- 6 months of a documented diet and exercise program
- Diagnosed obstructive sleep apnea (AHI of 15 or higher)
Qualification pathways
You can qualify through any one of these.
OSA Initial Therapy (age >= 18)
All of:
- Patient is >= 18 years of age
- Patient has a current BMI >= 30 kg/m2
- Sleep study within the past 1 year showing: diagnosis of moderate to severe OSA AND apnea-hypopnea index (AHI) >= 15 events per hour
- Patient does NOT have central sleep apnea with >= 50% central apneas/hypopneas
- Patient does NOT have Cheyne Stokes respiration
- Medication will be used concomitantly with behavioral modification and a reduced-calorie diet
OSA Continuation Therapy
All of:
- Patient is >= 18 years of age
- At baseline (before Zepbound), patient had a BMI >= 30 kg/m2
- Patient has completed >= 1 year of therapy with Zepbound
- Patient has lost >= 10% of baseline body weight
- Prescriber confirms stability in OSA signs or symptoms
- Medication will be used concomitantly with behavioral modifications and a reduced-calorie diet
Documentation to bring
- Documentation of current BMI >= 30 kg/m2
- Sleep study results within the past 1 year documenting: moderate to severe OSA diagnosis AND AHI >= 15 events per hour
- Documentation confirming absence of central sleep apnea (>= 50% central apneas/hypopneas) and absence of Cheyne Stokes respiration
- For continuation: documentation of baseline BMI >= 30 kg/m2 prior to Zepbound
- For continuation: documentation of >= 10% baseline body weight loss
- For continuation: prescriber attestation of stability in OSA signs or symptoms
Approval and renewal
- Initial approval: 12 months
- Renewal: every 12 months
- To renew, the plan looks for at least 10% weight loss from the starting weight and Patient must have completed >= 1 year of therapy with Zepbound AND lost >= 10% of baseline body weight AND have stability in OSA signs or symptoms (e.g., snoring, excessive daytime sleepiness, fatigue) per prescriber attestation. Make sure your starting weight and date are in the chart now — renewal is measured against it.
Not covered when
- Central sleep apnea with percent of central apneas/hypopneas >= 50%
- Cheyne Stokes respiration
- Concomitant use with other FDA-approved weight loss medications
- Concomitant use with other GLP-1 agonists or GLP-1/GIP agonists
Policy note: Sleep study must be within the past 1 year for initial approval. Moderate OSA defined as AHI >= 15 events/hour; severe OSA defined as AHI >= 30 events/hour. Initial and continuation approval both for 1 year. No PAP/CPAP trial requirement stated. Continuation requires both >= 10% body weight loss AND OSA symptom stability — higher weight loss threshold than the weight_loss indication (10% vs 5%). The OSA indication is subject to the same weight loss drug benefit restriction as all other indications in this policy.
