  • Age 18 and older
  • BMI of 32 or higher
  • BMI of 27 or higher with a weight-related condition (HTN, T2DM, dyslipidemia, OSA, CVD, knee osteoarthritis, asthma, COPD, NASH/NAFLD, PCOS, and CAD)

You can qualify through any one of these.

Adult Continuation Therapy (age >= 18)

All of:

  • Patient is >= 18 years of age
  • Patient has lost >= 5% of baseline body weight
  • Zepbound will be used concomitantly with a behavioral modification program and a reduced-calorie diet

  • At baseline, patient had a BMI >= 27 kg/m2 AND at least TWO weight-related comorbidities

Documentation to bring

  • Documentation of at least 3-month trial of behavioral modification and dietary restriction
  • For BMI >= 27 pathway: documentation of at least two qualifying weight-related comorbidities
  • For continuation: documentation of >= 5% baseline body weight loss
  • For continuation: prescriber attestation of ability to tolerate maintenance dose (5 mg, 10 mg, or 15 mg once weekly) or confirmation of ongoing dose titration toward 10 mg or 15 mg once weekly
  • Prescriber specialty credentials (to confirm not in excluded specialty)

Approval and renewal

  • Initial approval: 8 months
  • Renewal: every 12 months
  • To renew, the plan looks for at least 5% weight loss from the starting weight, Adult: must have lost >= 5% of baseline body weight. Patient must be able to tolerate a Zepbound maintenance dose of 5 mg, 10 mg, or 15 mg once weekly for 1-year approval; if still titrating to 10 mg or 15 mg and < 12 consecutive months received, approve for up to 4 months. Baseline BMI eligibility is re-verified at continuation, and staying on a maintenance dose of at least 5 mg. Make sure your starting weight and date are in the chart now — renewal is measured against it.

Not covered when

  • Concomitant use with other FDA-approved weight loss medications (e.g., phentermine, benzphetamine, diethylpropion, phendimetrazine, Contrave, Qsymia, orlistat)
  • Concomitant use with other GLP-1 agonists or GLP-1/GIP agonists
  • Prescriber who specializes primarily in: anesthesiology, dentistry, emergency medicine, nuclear medicine, ophthalmology, pathology, or radiology

Policy note: Zepbound weight loss indication applies to adults only (no pediatric criteria listed for Zepbound weight loss in this policy). Initial approval is 8 months. Continuation is 1 year if maintenance dose (5, 10, or 15 mg once weekly) is tolerated; up to 4 months if still titrating to 10 or 15 mg and <12 consecutive months completed. The policy notes that 5 mg once weekly should be achievable within the 8-month initial period. Baseline BMI refers to baseline prior to any GLP-1 or GLP-1/GIP agonist use.

  • Concomitant use with other FDA-approved weight loss medications
  • Concomitant use with other GLP-1 agonists or GLP-1/GIP agonists

Why Zepbound requests get denied by Medical Mutual

Based on what this policy asks for. Fix these before the first submission.

  1. Your specific plan doesn't include the weight loss benefit. Coverage here is conditional on the benefit design (often an optional employer add-on), so confirm it before submitting.
  2. Wrong brand for the diagnosis. Zepbound and Mounjaro are the same molecule with different approved uses; a request for Zepbound under a diagnosis that matches Mounjaro is routinely denied.
  3. BMI not documented in the chart notes (or documented without a baseline weight and date).
  4. Qualifying weight-related condition not documented with its own diagnosis code.
  5. No documented diet and exercise program.
  6. Baseline weight wasn't recorded at the start, so the required weight loss can't be shown at renewal.

If weight loss isn't covered on your plan

Medical Mutual has separate Zepbound policies that don't depend on the weight loss benefit:

  • Osa — covered on Employer / Commercial Insurance.

Zepbound is also sold directly by Eli Lilly without insurance through LillyDirect.
