What you need to qualify

  • Age 18 and older
  • BMI of 30 or higher
  • BMI of 27.5 or higher for patients of Asian descent
  • 6 months of a documented diet and exercise program
  • Diagnosed obstructive sleep apnea (AHI of 15 or higher), with a documented trial of PAP therapy

Qualification pathways

You can qualify through any one of these.

Initial approval for moderate to severe OSA

All of:

  • Diagnosis of moderate to severe obstructive sleep apnea
  • AHI/RDI/REI of at least 15 events per hour, predominantly obstructive
  • Age 18 or older
  • Actively enrolled in an exercise and caloric reduction program
  • Not using Zepbound concurrently with a GLP-1 receptor agonist

Plus any one of:

  • BMI >= 30 kg/m2
  • Asian patient with BMI >= 27.5 kg/m2

Initial OSA approval — PAP therapy condition

All of:

  • All other OSA initial criteria met

Plus any one of:

  • Persistent OSA symptoms despite PAP adherence (at least 4 hours per night on at least 70% of nights)
  • Clinical documentation that patient is not a candidate for PAP
  • Clinical documentation that patient is intolerant of PAP

Documentation to bring

  • Documentation of moderate to severe OSA diagnosis
  • AHI/RDI/REI result showing >= 15 events per hour that is predominantly obstructive
  • Documentation of BMI >= 30 kg/m2 (or >= 27.5 kg/m2 for Asian patients)
  • Documentation of active enrollment in exercise and caloric reduction program
  • Documentation of persistent OSA symptoms despite PAP adherence (>=4 hours/night on >=70% of nights) OR clinical documentation of ineligibility or intolerance to PAP
  • Confirmation that Zepbound will not be used concurrently with a GLP-1 receptor agonist

Approval and renewal

  • To renew, the plan looks for Patient must have achieved or maintained a reduction in AHI/RDI/REI by at least 15 events per hour OR by at least 50%. Concurrent GLP-1 receptor agonist use remains prohibited at renewal.

Not covered when

  • Concurrent use of a GLP-1 receptor agonist (liraglutide, semaglutide, exenatide, exenatide extended-release)

Policy note: OSA must be predominantly obstructive. PAP adherence is defined as at least 4 hours per night on at least 70% of nights. At renewal, AHI/RDI/REI must be reduced by at least 15 events per hour OR by at least 50% from baseline. Age and lifestyle program enrollment requirements from initial criteria are not explicitly re-verified at renewal per the renewal criteria text; only the AHI/RDI/REI improvement and no concurrent GLP-1 use are listed as renewal requirements.
