Initial approval — BMI >= 30 kg/m2 (non-Asian)
All of:

- Actively enrolled in exercise and caloric reduction program
- Not using Zepbound concurrently with a GLP-1 receptor agonist
Initial approval — Asian patient BMI >= 27.5 kg/m2
All of:
- Age 18 or older
- Asian patient with BMI >= 27.5 kg/m2
- Actively enrolled in exercise and caloric reduction program
- Not using Zepbound concurrently with a GLP-1 receptor agonist
Initial approval — BMI >= 27 kg/m2 with comorbidity (non-Asian)
All of:
- Age 18 or older
- BMI >= 27 kg/m2
- At least one weight-related comorbidity (hypertension, T2DM, dyslipidemia, cardiovascular disease, CAD, sleep apnea, osteoarthritis of the knee, polycystic ovarian syndrome, NASH/NAFLD, asthma, COPD)

Documentation to bring
- Baseline BMI documentation (>= 30 kg/m2, or >= 27.5 kg/m2 for Asian patients, or >= 27 kg/m2 with comorbidity, or >= 25 kg/m2 for Asian patients with comorbidity)
- Documentation of at least one weight-related comorbidity if using lower BMI threshold pathway
- Documentation of active enrollment in exercise and caloric reduction program
- Confirmation that Zepbound will not be used concurrently with a GLP-1 receptor agonist
- At renewal: documentation of at least 5% weight loss from baseline body weight
Approval and renewal
- To renew, the plan looks for at least 5% weight loss from the starting weight and Patient must have achieved or maintained at least 5% weight loss from baseline (pre-treatment) body weight. BMI eligibility thresholds are re-verified at renewal (same thresholds as initial). Age and lifestyle program enrollment requirements from initial criteria are not explicitly re-verified at renewal. Concurrent GLP-1 use remains prohibited. Make sure your starting weight and date are in the chart now — renewal is measured against it.
Not covered when
- Concurrent use of a GLP-1 receptor agonist (liraglutide, semaglutide, exenatide, exenatide extended-release)
Policy note: For Asian patients, the BMI threshold without comorbidity is 27.5 kg/m2 and with comorbidity is 25 kg/m2. For non-Asian patients, BMI threshold without comorbidity is 30 kg/m2 and with comorbidity is 27 kg/m2. At renewal, the BMI eligibility thresholds are re-verified using the same criteria as initial approval, plus the 5% weight loss requirement. The lifestyle program enrollment and age requirement are present in initial criteria but not explicitly listed in renewal criteria.
