What you need to qualify

  • BMI of 30 or higher
  • Diagnosed obstructive sleep apnea (AHI of 15 or higher)

Qualification pathways

You can qualify through any one of these.

OSA with obesity (Zepbound)

All of:

  • Diagnosis of obstructive sleep apnea (OSA) (medical records required)
  • Polysomnography (PSG) or home sleep apnea test performed (sleep study required)
  • AHI >= 15 events/hour from baseline (prior to initiation of pharmacotherapy) (sleep study required)
  • Requested agent is Zepbound
  • BMI >= 30 kg/m2 (medical records required)
  • Will NOT use in combination with another weight loss agent
  • Will NOT use in combination with another GLP-1 receptor agonist
  • No FDA labeled contraindications to requested agent
  • Currently on low-calorie diet, increased physical activity, and behavioral modifications

Documentation to bring

  • Medical records confirming diagnosis of obstructive sleep apnea (OSA)
  • Sleep study results (polysomnography or home sleep apnea test) showing AHI >= 15 events/hour at baseline
  • Medical records confirming BMI >= 30 kg/m2

Quantity limits

  • 2.5mg — 4 vials or 4 pens per 180 days (84-day)
  • 5mg, 7.5mg, 10mg, 12.5mg, 15mg — 4 vials or 4 pens per 28 days

Approval and renewal

  • Initial approval: 6 months
  • Renewal: every 6 months
  • To renew, the plan looks for Patient must have had clinical benefit with the requested agent (e.g., reduction in AHI, decrease in Epworth Sleepiness Scale). OSA diagnosis and AHI/BMI requirements are NOT re-verified at renewal — only clinical benefit confirmation is required.

Not covered when

  • Concomitant use with another weight loss agent (e.g., Contrave, phentermine, Qsymia, Xenical)
  • Concomitant use with another GLP-1 receptor agonist
  • Any FDA labeled contraindications to the requested agent

Policy note: PAP therapy trial is NOT required prior to approval — the policy does not mandate a prior PAP/CPAP trial. Both patients currently on PAP and those not on PAP are eligible (consistent with SURMOUNT-OSA trial design referenced in the document). AHI >= 15 events/hour corresponds to moderate-to-severe OSA. Initial approval is 6 months (non-obesity indication). Renewal is 6 months with clinical benefit required (reduction in AHI or decrease in Epworth Sleepiness Scale).
