AHI ≥ 15 events/hour from baseline (prior to initiation of pharmacotherapy) (sleep study required)
Requested agent is Zepbound
BMI ≥ 30 kg/m2 (medical records required)
Will NOT use in combination with another weight loss agent
Will NOT use in combination with another GLP-1 receptor agonist
No FDA labeled contraindications to requested agent
Currently on low-calorie diet, increased physical activity, and behavioral modifications

Both patients currently on PAP and those not on PAP are eligible (consistent with SURMOUNT-OSA trial design referenced in the document). AHI ≥ 15 events/hour corresponds to moderate-to-severe OSA. Initial approval is 6 months (non-obesity indication). Renewal is 6 months with clinical benefit required (reduction in AHI or decrease in Epworth Sleepiness Scale).
