- 3 months of a documented diet and exercise program
- Prior trial of another GLP-1 medication (Foundayo) for at least 180 days
- Diagnosed obstructive sleep apnea
Qualification pathways
You can qualify through any one of these.
OSA in adults with obesity — integrated into weight management criteria (Class III obesity)
All of:
- Age ≥ 18 years
- Diagnosis of moderate to severe obstructive sleep apnea
- Obesity (BMI ≥ 40 kg/m²)
- Enrolled in qualified comprehensive weight management program for ≥3 consecutive months
- Documentation of trial and failure of Foundayo OR documented contraindication to Foundayo
- Not used in combination with any other GIP and/or GLP-1 receptor agonist
- Coverage criteria apply to Zepbound pens only (vials and Kwikpens not covered)
OSA in adults with obesity — integrated into weight management criteria (Class II obesity with OSA comorbidity)
All of:
- Age ≥ 18 years
- Diagnosis of moderate to severe obstructive sleep apnea
- BMI 35–39.9 kg/m²
- OSA as qualifying comorbidity
- Enrolled in qualified comprehensive weight management program for ≥3 consecutive months
- Documentation of trial and failure of Foundayo OR documented contraindication to Foundayo
- Not used in combination with any other GIP and/or GLP-1 receptor agonist
- Coverage criteria apply to Zepbound pens only (vials and Kwikpens not covered)
Documentation to bring
- Baseline BMI and weight documentation
- Documentation of moderate to severe obstructive sleep apnea diagnosis
- Documentation of obesity (BMI ≥35 kg/m² with OSA as comorbidity, or BMI ≥40 kg/m²)
- Documentation of current enrollment in qualified comprehensive weight management program for ≥3 consecutive months
- Proof of participation in comprehensive weight management program (receipt or certificate and dietary/exercise logs)
- Documentation of trial and failure of Foundayo OR documented contraindication to Foundayo
Quantity limits
- 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg, 15mg — 2 mL (4 pens) per 28 days; Maximum 14 twenty-eight-day supplies OR 5 eighty-four-day supplies per 365 days; first 6 fills for new starts limited to 28-day supply or less; 2.5mg strength limited to 2mL per 365 days (titration only) (84-day)
Approval and renewal
- Initial approval: 7 months
- Renewal: every 6 months
- To renew, the plan looks for at least 5% weight loss from the starting weight, OSA indication uses same continuation criteria as weight management: ≥5% weight loss by 7 months at initial recertification; maintained ≥5% weight loss or continued weight loss at ongoing 6-month recertifications; BMI not less than 18.5 kg/m²; maintenance dose (5mg, 7.5mg, 10mg, 12.5mg, or 15mg) required at recertification. OSA diagnosis not re-verified at continuation, at least 17 weeks on therapy before the first renewal, and staying on a maintenance dose of at least 5 mg. Make sure your starting weight and date are in the chart now — renewal is measured against it.
Not covered when
- Zepbound vials and Kwikpens are not a covered benefit for this indication
- Combination with other GIP and/or GLP-1 receptor agonists not approved
- Combination with any other weight loss drugs (Rx, OTC, or herbal) not approved
- 2.5mg strength limited to initial 4-week titration only
- Clinical efficacy in OSA determined to be inherently dependent on weight loss — OSA indication integrated into weight management criteria
Exceptions
- New York: All utilization management requirements compliant with applicable New York State insurance laws and regulations
Policy note: FDA approved Zepbound for moderate to severe OSA in adults with obesity on December 20, 2024. Plan has integrated OSA indication into existing weight management criteria because clinical efficacy in OSA is determined to be inherently dependent on weight loss. No specific AHI threshold is stated — policy refers to 'moderate to severe OSA' consistent with FDA-approved indication. No PAP therapy trial requirement specified. OSA qualifies as a comorbidity for Class II obesity coverage threshold. AHI minimum not quantified in this policy document.
