What you need to qualify

  • Age 18 and older
  • BMI of 30 or higher
  • Diagnosed obstructive sleep apnea (AHI of 15 or higher)

Qualification pathways

You can qualify through any one of these.

Moderate-to-severe OSA in adults with obesity

All of:

  • Age >= 18 years
  • BMI >= 30 kg/m2 (obesity)
  • Diagnosis of moderate-to-severe OSA confirmed by PSG or HSAT (AHI/REI >= 15 events/hour for moderate; >= 30 for severe)

Plus any one of:

  • Not currently receiving PAP therapy (can receive Zepbound with or without PAP per SURMOUNT-OSA trials)
  • Currently receiving PAP therapy (SURMOUNT-OSA Trial 2 included PAP users)

Documentation to bring

  • Diagnosis of moderate-to-severe OSA confirmed by polysomnography (PSG) or home sleep apnea test (HSAT) with AHI or REI >= 15 events/hour
  • Baseline BMI documentation >= 30 kg/m2
  • Sleep study results (PSG or HSAT report) documenting AHI/REI severity
  • Documentation of reduced-calorie diet and increased physical activity as adjunct

Not covered when

  • Craniofacial abnormalities affecting respiration (per SURMOUNT-OSA exclusion criteria cited in document)
  • Predominantly central or mixed sleep apnea (>= 50% central or mixed apneas/hypopneas)
  • Cheyne-Stokes respiration
  • Obesity hypoventilation syndrome
  • Daytime hypercapnia
  • Coadministration with other tirzepatide-containing products not recommended
  • Coadministration with any GLP-1 receptor agonist not recommended
  • Pregnancy

Policy note: OSA indication requires obesity (BMI >= 30) per FDA label. Moderate-to-severe OSA defined as AHI >= 15 events/hour (moderate 15-29, severe >= 30). The SURMOUNT-OSA program enrolled both PAP-naive (Trial 1) and PAP-using (Trial 2) patients; PAP trial is NOT required as a prerequisite.
