Medical Mutual coverage for Wegovy (Weight Loss)

Prior Authorization is only available for members with plan benefit coverage of weight loss drugs. Medications for the treatment of weight loss are specifically excluded as a covered benefit for many plans.

What you need to qualify

  • Age 12 and older
  • BMI of 32 or higher
  • BMI of 27 or higher with a weight-related condition (HTN, T2DM, dyslipidemia, OSA, CVD, knee osteoarthritis, asthma, COPD, NASH/NAFLD, PCOS, and CAD)
  • BMI at or above the 95th percentile for age and sex
  • 3 months of a documented diet and exercise program

Qualification pathways

You can qualify through any one of these.

Adult Initial Therapy (age >= 18)

All of:

  • Patient is >= 18 years of age
  • Patient has engaged in a trial of behavioral modification and dietary restriction for at least 3 months
  • Wegovy will be used concomitantly with a behavioral modification program and a reduced-calorie diet
  • Prescriber does not specialize primarily in: anesthesiology, dentistry, emergency medicine, nuclear medicine, ophthalmology, pathology, or radiology

Plus any one of:

  • At baseline, patient had a BMI >= 32 kg/m2
  • At baseline, patient had a BMI >= 27 kg/m2 AND at least TWO of the following weight-related comorbidities: hypertension, type 2 diabetes, dyslipidemia, OSA, CVD, knee osteoarthritis, asthma, COPD, NASH/NAFLD, PCOS, or CAD

Adult Continuation Therapy (age >= 18)

All of:

  • Patient is >= 18 years of age
  • Patient has lost >= 5% of baseline body weight
  • Wegovy will be used concomitantly with a behavioral modification program and a reduced-calorie diet
  • Prescriber does not specialize primarily in: anesthesiology, dentistry, emergency medicine, nuclear medicine, ophthalmology, pathology, or radiology
  • Patient has completed at least 7 months of initial therapy OR is within initial approval period

Plus any one of:

  • At baseline, patient had a BMI >= 32 kg/m2
  • At baseline, patient had a BMI >= 27 kg/m2 AND at least TWO weight-related comorbidities

Pediatric Initial Therapy (age 12 to <18)

All of:

  • Patient is >= 12 years of age and < 18 years of age
  • Patient has engaged in a trial of behavioral modification and dietary restriction for at least 3 months
  • At baseline, patient had a BMI >= 95th percentile for age and sex
  • Wegovy (injectable only) will be used concomitantly with behavioral modification and a reduced-calorie diet
  • Prescriber does not specialize primarily in: anesthesiology, dentistry, emergency medicine, nuclear medicine, ophthalmology, pathology, or radiology

Pediatric Continuation Therapy (age 12 to <18)

All of:

  • Patient is >= 12 years of age and < 18 years of age
  • At baseline, patient had a BMI >= 95th percentile for age and sex
  • Patient has had a reduction in BMI of >= 1% from baseline
  • Wegovy (injectable only) will be used concomitantly with behavioral modification and a reduced-calorie diet
  • Prescriber does not specialize primarily in: anesthesiology, dentistry, emergency medicine, nuclear medicine, ophthalmology, pathology, or radiology
  • Patient has completed at least 7 months of initial therapy OR is within initial approval period

Plus any one of:

  • Patient is able to tolerate a Wegovy maintenance dose of 1.7 mg or 2.4 mg once weekly (approve for 1 year)
  • Patient has received < 12 consecutive months of Wegovy AND prescriber confirms patient is continuing to titrate to target dose (approve up to 5 months)

Documentation to bring

  • Documentation of baseline BMI (>= 32 kg/m2, OR >= 27 kg/m2 with at least two qualifying comorbidities) prior to any GLP-1 or GLP-1/GIP agonist use
  • Documentation of at least 3-month trial of behavioral modification and dietary restriction
  • For BMI >= 27 pathway: documentation of at least two qualifying weight-related comorbidities
  • For pediatric patients: documentation of baseline BMI >= 95th percentile for age and sex
  • For continuation: documentation of >= 5% baseline body weight loss (adults) or >= 1% BMI reduction from baseline (pediatric)
  • For continuation: prescriber attestation of ability to tolerate maintenance dose or confirmation of ongoing dose titration
  • Prescriber specialty credentials (to confirm not in excluded specialty)

Approval and renewal

  • Initial approval: 7 months
  • Renewal: every 12 months
  • To renew, the plan looks for at least 5% weight loss from the starting weight, at least 1% reduction in BMI, Adult: must have lost >= 5% of baseline body weight. Pediatric (12-<18): must have had a reduction in BMI of >= 1% from baseline. Patient must be able to tolerate injectable maintenance dose of 1.7 mg or 2.4 mg once weekly OR oral maintenance dose of 25 mg once daily for 1-year approval; alternatively, if still titrating and <12 consecutive months received, approve for up to 5 months. Baseline BMI eligibility is re-verified at continuation, and staying on a maintenance dose of at least 1.7 mg. Make sure your starting weight and date are in the chart now — renewal is measured against it.

Not covered when

  • Concomitant use with other FDA-approved weight loss medications (e.g., phentermine, benzphetamine, diethylpropion, phendimetrazine, Contrave, Qsymia, orlistat)
  • Concomitant use with other GLP-1 agonists or GLP-1/GIP agonists
  • Prescriber who specializes primarily in: anesthesiology, dentistry, emergency medicine, nuclear medicine, ophthalmology, pathology, or radiology

Policy note: Annual Review Date listed as 07/17/2025; Last Revised Date 01/15/2026. Wegovy is available as oral or injectable for adult weight loss; injectable only for pediatric weight loss. Baseline BMI refers to baseline prior to any GLP-1 or GLP-1/GIP agonist use. Coverage is generally not available unless the member's plan includes weight loss drug benefits. Initial approval for adults is 7 months; continuation approval is up to 1 year if maintenance dose tolerated, or up to 5 months if still titrating (with sufficient duration to reach 12 consecutive months total).
