Documentation to bring
- Completed existing 'WEGOVY for MACE (Major Adverse Cardiovascular Events)' prior authorization form
- Valid ICD-10-CM diagnosis code indicating established major adverse cardiovascular event (from ctdssmap.com Pharmacy ICD-10-CM-Therapeutic Class Product Diagnosis List)
