What you need to qualify

• Age 45 and older
• BMI of 27 or higher
• Established cardiovascular disease (CV death (established CVD), nonfatal MI, and nonfatal stroke)
• Not being used to treat type 1 diabetes

Documentation to bring

• Documentation of established cardiovascular disease (history of CV death risk, nonfatal MI, or nonfatal stroke)
• BMI documentation >= 27 kg/m2
• Age >= 45 years
• Confirmation of absence of diabetes (SELECT trial excluded patients with diabetes)

Not covered when

• Concomitant use with other semaglutide-containing products or any other GLP-1 receptor agonist is not recommended
• SELECT trial enrolled patients without diabetes; diabetes status should be confirmed absent for this indication

Policy note: Indication is to reduce risk of major adverse CV events (CV death, nonfatal MI, or nonfatal stroke) in adults with established CV disease and either obesity or overweight. The SELECT trial (NCT03574597) enrolled adults >= 45 years with BMI >= 27 kg/m2 and established CVD without diabetes. The document explicitly notes the SELECT trial population was 'without diabetes', supporting excludes_diabetes: true for this indication. Standard cardiovascular care should be ongoing.
