- 6 months of a documented diet and exercise program
Qualification pathways
You can qualify through any one of these.
Adult obesity without comorbidity
All of:
- BMI >= 30 kg/m2
- Inadequate response to comprehensive lifestyle intervention program
Adult overweight with comorbidity
All of:
- BMI >= 27 kg/m2
- Inadequate response to comprehensive lifestyle intervention program
Plus any one of:
- Hypertension
- Type 2 diabetes
- Dyslipidemia
- Obstructive sleep apnea
- Cardiovascular disease
- Other weight-related comorbid condition
Documentation to bring
- Documentation of BMI >= 30 kg/m2 or >= 27 kg/m2 with at least one weight-related comorbidity
- Documentation of prior comprehensive lifestyle intervention program attempt or reason it is not feasible
- Documentation of weight-related comorbid condition if BMI is 27-29.9 kg/m2
Approval and renewal
- To renew, the plan looks for at least 5% weight loss from the starting weight, Per referenced guidelines, if weight loss >= 5% of body weight at 3 months, medication should be continued. If weight loss < 5% at 3 months or safety/tolerability issues, medication should be discontinued, and at least 12 weeks on therapy before the first renewal. Make sure your starting weight and date are in the chart now — renewal is measured against it.
Not covered when
- Coadministration with other tirzepatide-containing products is not recommended
- Coadministration with any GLP-1 receptor agonist is not recommended
