What you need to qualify
- Age 18 and older
- BMI of 27 or higher
- Established cardiovascular disease (MI, stroke, and PAD)
- On guideline-directed medical therapy (optimized pharmacotherapy for established cardiovascular disease)
Qualification pathways
You can qualify through any one of these.
CV risk reduction — established cardiovascular disease with qualifying event
All of:
- Patient is 18 years of age or older
- Using medication to reduce risk of MACE (cardiovascular death, non-fatal MI, or non-fatal stroke)
- Established cardiovascular disease (defined as: CAD, ACS, history of MI, stable or unstable angina, coronary or other arterial revascularization, prior PCI/CABG, stroke, TIA, carotid or other arterial stenosis, or PAD including aortic aneurysm, all of atherosclerotic origin)
- BMI >= 27 kg/m2
- Patient will use optimized pharmacotherapy for established cardiovascular disease in combination with Wegovy
- Requested medication will NOT be used concurrently with another GLP-1 receptor agonist
- Requested medication will NOT be used concurrently with another weight loss medication
- Patient has anti-obesity medication coverage benefits
Plus any one of:
- History of myocardial infarction
- History of stroke
- Peripheral artery disease defined by intermittent claudication with ankle-brachial index < 0.85 at rest, peripheral arterial revascularization procedure, or amputation due to atherosclerotic disease
Documentation to bring
- Documentation of established cardiovascular disease (CAD, ACS, MI, angina, revascularization, PCI/CABG, stroke, TIA, carotid/arterial stenosis, or PAD of atherosclerotic origin)
- Documentation of qualifying CV event history (MI, stroke, or PAD with ABI < 0.85, revascularization, or amputation)
- Baseline BMI >= 27 kg/m2
- Documentation of current use of optimized pharmacotherapy for established cardiovascular disease or clinical reason it is not appropriate
- Documentation that patient is not concurrently using another GLP-1 receptor agonist
- Documentation that patient is not concurrently using another weight loss medication
- Confirmation of anti-obesity medication coverage benefits
Quantity limits
- 0.25 mg/0.5 mL, 0.5 mg/0.5 mL, 1 mg/0.5 mL — 4 pens per 28 days
- 1.5 mg tablet — 1 tablet per day
- 4 mg tablet — 1 tablet per day
- 9 mg tablet — 1 tablet per day
Approval and renewal
- Initial approval: 6 months
- Renewal: every 12 months
- To renew, the plan looks for Patient must continue optimized pharmacotherapy for established CVD in combination with Wegovy AND demonstrate a positive clinical response. Initial CVD, BMI, and GDMT requirements are re-verified at continuation for this indication.
Not covered when
- Concurrent use of another GLP-1 receptor agonist
- Concurrent use of another weight loss medication
Policy note: Applies to both Wegovy injection (0.25mg–2.4mg) and Wegovy tablets. CV risk reduction indication requires established CVD with at least one specific qualifying event (MI, stroke, or PAD) plus BMI >= 27. 'Optimized pharmacotherapy' is not further specified in the policy. Excludes_diabetes is false — the policy does not exclude diabetic patients from this indication.
