- Not being used to treat type 1 diabetes
Qualification pathways
You can qualify through any one of these.
CV risk reduction in adults with established CVD and overweight/obesity without diabetes
All of:
- Age >= 45 years
- Established cardiovascular disease
- BMI >= 27 kg/m2
- No diabetes diagnosis (SELECT trial enrolled patients without diabetes)
Plus any one of:
- History of non-fatal MI
- History of non-fatal stroke
- Other established CV disease
Documentation to bring
- Documentation of established cardiovascular disease (CV death history, non-fatal MI, non-fatal stroke, or equivalent)
- Baseline BMI documentation >= 27 kg/m2
- Confirmation of absence of diabetes diagnosis
- Age >= 45 years confirmed
Not covered when
- Patients WITH diabetes were excluded from the SELECT trial; CV indication inferred for non-diabetic patients only
- Concomitant use with other semaglutide-containing products not recommended
- Concomitant use with any other GLP-1 receptor agonist not recommended
- Pregnancy
Policy note: CV risk reduction indication supported by SELECT trial data (NCT03574597): semaglutide 2.4 mg reduced MACE by 20% vs placebo in patients >= 45 years with BMI >= 27 and established CVD WITHOUT diabetes. The policy document references this trial to support the indication. Age minimum of 45 is derived from SELECT trial enrollment criteria referenced in the document. The FDA label indication is for adults with established CV disease and obesity or overweight, but the document specifically references the non-diabetic population from SELECT.
